Health Savings Account (HSA) Application

Here's what you will need:

• To be 18 years of age or older. If under the age of 18, please call us at 1-877-275-2228 or visit your closest branch location.
• Your Social Security Number / Individual Taxpayer Identification Number, verify any credit freezes on your Transunion Credit Report have been lifted prior to submitting your application.
• Government Issued Identification for all Applicants / Authorized Signers. You will be asked to upload clear / color images of these documents.
• If funding your new accounts by electronic transfer from another financial institution, you’ll need their routing number and your account number. If funding with a debit or credit card, you’ll need the card number, expiration date, and CVV code.
• New Members of CCU are required to open a Membership Savings Account, this will be completed during this application process.

General HSA Guidelines / Information:

• You must be covered under a high deductible health plan (HDHP) the first day of this month.
• You can only have health coverage as permitted by the IRS.
• You cannot be claimed as a dependent on any tax return.
• The 2024 HSA contribution limits are $4,150 for Single-coverage and $8,300 for Family-coverage.
